Instructions
1. Click the words Ministry Form above to access the form in MS Word. Internet Explorer: You can fill out the form on the internet as it appears. Your browser will still show and you can now see a ruler at the top of the document as if it is in MS Word. Netscape Navigator and Mozilla Firefox: You can choose to open the form in Word. There will be no browser, just Word.
2. This is a table form that you can fill out without printing a hard copy. Do not change the words in the bold (yellow areas). Just fill in your responses in the clear spaces in the table.
3. After you are done, save this form with its original name or another name. Click the link at the end of the form to open your e-mail program so that you can send it to the Healing Rooms.
4. Depending on your e-mail program, attach your saved form by locating it in a directory on your computer. Send it as an attachment to the e-mail message. You are done!
Notes: Internet Explorer: You can click File/Send/Page by-Email or Link by E-mail. Make sure you are sending from your correct e-mail program when doing this. If your document opened up in Word only, you can click File/Send To and choose the Mail Recipient or another choice on the menu. Your form must be saved so that you can attach it to the e-mail.
